Your role as a Homage NDIS Support Coordinator is essential in helping those with NDIS funding to maximise their package and have access to the support and services they need to live independently.   • Understanding the care and support that can be accessed with funding.
  • Understanding NDIS terminology and the NDIS pricing guide.
  • Finding service providers to facilitate the care and support needed to ensure the participant retains choice and control of their providers.
  • Negotiating with service providers on the participants behalf.
  • Coordinating and implementing support in the NDIS participant’s plan to ensure they can live an enriched and fulfilling life.
  • Building capacity and empowering participants with access to tools and resources to manage their own care and support more independently over time.
  • Resolving conflicts with service providers.
  • Using the myGov website and NDIS participant portal.
  • Planning for their NDIS plan review.

What We Look For in a Homage NDIS Support Coordinator

With a high level understanding of NDIS and NDIS practice standards, you will have strong communication and interpersonal skills. Building a strong rapport with internal and external stakeholders, you will have experience in handling complex situations and be able to resolve issues. Able to work under pressure, you will be a meticulous, organised self motivator with attention to detail.

Additionally you will have:

  • Minimum of 2 years experience working as a Support Coordinator.
  • Diploma (or higher) in Community Service or similar.
  • Valid drivers licence, First Aid Certificate, Working with Children Check and NDIS Worker Screening check.
  • Australian Business Number (ABN) or be willing to obtain one
  • Experience in case management coordinating individual plans.
  • Previous experience working in a start-up high growth environment is desirable.

How much work is available with Homage? What are my obligations to work with Homage?

As an NDIS Support Coordinator at Homage you will enjoy full flexibility to manage your own caseload and take on the number of participants that your schedule allows you to.
When working with Homage, you will be contacted directly when there are clients available in your area who require Support Coordination services. After an in person or zoom call “Meet and Greet” session with the participant, you will be assigned to provide support on an ongoing basis.
You will be required to record case notes and time you spend with participants so Homage can appropriately bill clients/Plan Managers and pay you for your services.
